For a bigger problem that needs data and a real project, not a quick fix. It pulls in 5 Whys, Fishbone, and Pareto as you go.

How to run it

Work the phases in order, set the current stage as you progress, and do not move on until each phase has an honest answer.

Worked example: flatbed reprints

Define: reprints at 4% cost 6% of flatbed hours; target under 2%. Measure: baseline 4.1 per 100 jobs from the reprint log. Analyze: Pareto shows color drift drives 60%; root cause is no color-target sign-off. Improve: add a sign-off; pilot drops it to 1.6. Control: sign-off is now standard, the lead operator reviews the log weekly.
