An A3 carries a problem from background all the way to follow-up on a single sheet. The left side builds understanding, the right side commits to the fix. Filling it forces the clarity that gets a whole team aligned on one story instead of six opinions.
What it is
A single page, named for the paper size, that tells a problem's whole story: background, current state, goal, root cause, countermeasures, plan, and follow-up.
When to use it
To align a team on a meaningful problem, to propose a fix to a manager, or to train. It is the report your 5 Whys, Fishbone, and Pareto work feeds into.
How to run it
Work the left side until the root cause is clear, then commit the right side to specific countermeasures, owners, and a follow-up date.

Worked example: reprints on the flatbed line
Left side: reprints cost the shop 6% of flatbed hours; current rate is 4 per 100 jobs; target is under 2 by quarter end; root cause is no color-target sign-off at makeready. Right side: add a sign-off step, owner is the lead operator, due in two weeks, follow up by reviewing reprint logs at the next month-end. One page, one story, one aligned team.
