Ops Console / Module 07 / RACI
One task, one owner, no dropped balls.
Most dropped work is not laziness, it is unclear ownership. RACI assigns every task a Responsible doer and exactly one Accountable owner, plus who to Consult and Inform. This builder maps it on a grid and checks that every task has a single clear owner, the rule that quietly prevents most things from falling through.

For any process or project where "I thought you had it" happens. The rule is one, and only one, Accountable per task, the single throat to choke.
List the tasks and the roles, then mark each cell. The check flags any task that does not have exactly one Accountable, the most common source of dropped balls.
R Responsible does the work · A Accountable owns the outcome (exactly one per task) · C Consulted gives input · I Informed is kept in the loop.
Tasks run from take the order to ship it; roles are sales, designer, press op, and finisher. Marking the grid quickly exposes the proofing row, where both sales and the designer are marked Accountable. Two owners means no owner. A quick conversation makes the designer accountable and sales consulted, and the dropped proofs stop.
